PhilHealth ID : Step-by-step application

PhilHealth is a government agency that provides healthcare insurance to Filipinos. The PhilHealth ID serves as proof of membership and can be used to avail of healthcare services. With the ongoing pandemic, it is essential to have a PhilHealth ID for easier access to medical benefits. This article will provide you with a step-by-step guide on how to apply for a PhilHealth ID online and the requirements needed.


Eligibility Requirements

With the passage of the Universal Health Care (UHC) Act or Republic Act (RA) 11223 and a mandate to provide health insurance coverage and ensure affordable, acceptable, available and accessible health care services for all citizens of the Philippines, membership to Philhealth Program was categorized into two (2):


1. Direct Contributors

2. Indirect Contributors


As per Sec. 4.8 of the UHC Act, Direct contributors refer to those who have the capacity to pay premiums, are gainfully employed and are bound by an employer-employee relationship, or are self-earning, professional practitioners, migrant workers, including their qualified dependents, and lifetime members.


Sec. 8.2 of the Act enumerated an unexhaustive list of direct contributors, including their qualified dependents:

  • Employed and registered under the Philippine Social Security System (SSS) for those employees working in the private sector or Government Service Insurance System (GSIS) for those employees working in the public sector.

  • Self-earning Filipino aged 21 years and above

  • Kasambahay per RA 10361 (Domestic Workers Act)

  • Overseas Filipino Worker per RA 10022 (Migrant Workers Act) and RA 10801 (OWWA Act) either Sea-based workers/seafarers or Land-based workers

  • Filipino living abroad

  • Filipino with dual citizenship

  • Lifetime members means being a retiree or pensioner per RA 10606 (National Health Insurance Act)

While as per Sec. 4.19 of the Act, Indirect contributors refer to all others not included as direct contributors, as well as their qualified dependents, whose premium shall be subsidized by the national government including those who are subsidized as a result of special laws.


Likewise, Sec. 8.4 of the Act enumerated an unexhaustive list of indirect contributors, including their qualified dependents:

  • Indigents identified by the DSWD;

  • Beneficiaries of Pantawid Pamilyang Filipino Program (4Ps) & Modified Conditional Cash Transfer (MCCT);

  • Senior citizens who are not currently covered by the Program;

  • Persons with disability per RA 10754 (An Act Expanding the Benefits and Privileges of Persons with Disability);

  • All Filipinos aged 21 years old and above without the capacity to pay the premiums;

  • Sangguniang Kabataan officials, as defined in R.A. 10742 (Sangguniang Kabataan Reform Act); and,

  • Filipinos previously identified at point-of-service (POS) or during registration, members previously sponsored by LGUs and those who are not yet in the PhilHeaith database and are financially incapable to pay premiums.

As per Sec 8.1 of the similar Act, the qualified dependents of either the direct or indirect contributors are also covered by Philheath namely:

  • Legal spouse who is not an active member;

  • Unmarried and unemployed legitimate, illegitimate children, and legally adopted or stepchildren below 21 years of age;

  • Foster children as per RA 10165 (Foster Care Act of2012); and,

  • Parents who are sixty (60) years old and above, not otherwise an enrolled member


PhilHealth ID Application Requirements

For new members or first-time registrants, there are two (2) options available in applying for your Philhealth ID no.:


1. You may visit the nearest Philhealth office and bring the following requirements in applying for a PhilHealth ID:

  • Duly accomplished Philhealth Member Registration Form (PMRF)

  • Two copies of latest 1×1 ID pictures

  • At least two valid IDs e.g. SSS ID, TIN, Passport etc.

  • A photocopy of your PSA-issued Birth Certificate and/or Marriage Certificate, for married-female registrant.

  • In case with qualified dependent, a photocopy of their PSA-issued Birth Certificate

Wait for the processing of your Member’s Data Record (MDR) and PhilHealth which will be released the same day. Do not forget to double-check all the information before leaving.


2. You may send an email to actioncenter@philhealth.gov.ph and attached the scanned copy of the following requirements:

  • Duly accomplished Philhealth Member Registration Form (PMRF)

  • At least two valid IDs e.g. SSS ID, TIN, Passport etc.

  • A photocopy of your PSA-issued Birth Certificate and/or Marriage Certificate, for married-female registrant.

  • In case with qualified dependent, a photocopy of their PSA-issued Birth Certificate

Follow below format on the email Subject line:

Register <space> (Full Name) <space> (City or Province) <space> (Region)

Example: Register Maria Makiling Manila, NCR


Philhealth will process your application. Through email, Philhealth will send your Philhealth ID No. to the email address you indicated on the PMRF.

What’s inside PhilHealth Member Registration Form (PMRF)?

To register or update/amend the membership record with Philhealth, a form called PMRF is to be properly accomplished and shall be accompanied by a valid proof of identity for first time registrants, and supporting documents to establish relationship between member and dependent/s for updating or request for amendment.

Once all information and supporting documents are correct. affix your signature on the form. Remember you are signing under the penalty of law.


In case you forgot, how to know your PhilHealth Number?

To know your PhilHealth Number, there are four ways to access it:

  1. Member's Data Record (MDR). Your PhilHealth number is printed on your MDR.

  2. Call PhilHealth customer service. You can call the PhilHealth hotline at (02) 8441-7442 to request your PhilHealth number. Apart from calling the said hotline, you can also reach out to PhilHealth’s Corporate Action Center through its callback channel 0921-630-0009. Be ready to provide your personal information such as your full name, birthdate, and address to confirm your identity.

  3. Check your payslip or any documents from your employer. Your PhilHealth number may also be printed on your payslip or any documents from your employer, especially if they deduct PhilHealth contributions from your salary. Try reaching out to your previous or present company's HR Department.

  4. Visit the nearest PhilHealth office. If all else fails, you can visit the nearest PhilHealth office and request for your PhilHealth number. Make sure to bring a valid ID and any document that can verify your identity, like a birth certificate or passport.

How to replace Lost or Stolen or Damaged Philhealth ID Card?

Assuming you knew your PIN, to process the replacement of your lost or stolen or damaged Philhealth ID card, you have to go to your nearest Philhealth Office and bring at least two valid IDs.


  • In case of lost or stolen ID, a notarized Affidavit of Loss is required to be presented to the Philheatlh office.

  • In case of damaged ID, you need to surrender such ID card to the Philhealth office.

For both cases, you may need to submit a request form which is available at the Philhealth office.


Your PhilHealth Identification Number (PIN) is your unique and permanent number. You will always be required to use your PIN in case you will have transactions with PhilHealth. It is also a valid government ID in case of transactions with government offices and other private entities.
